Health Program Offered at the Woburn Senior Center

The Woburn Senior Center, 144 School Street, offers a way for seniors to receive an extra measure of healthcare, free of charge.  The VNA of Middlesex-East offers a HomMed Health Center kiosk at the Senior Center to help seniors better manage their health care status and maintain an independent lifestyle longer. 

The kiosk contains a built-in telemonitor that allows individuals to take their own vital signs and securely transmits them to a main station located at the VNA of Middlesex-East main office in Wakefield.  Agency nurses review this data daily and follow-up with individuals if they see changes in their vital signs that warrant attention.  People are able to access the telemonitor by sliding a personalized card through a card reader.  The system guides users through the process of collecting health status information (blood pressure, heart rate, weight, and oxygen saturation) with easy to follow voice and text prompts.  The monitoring of vital signs allows clinicians to catch health problems early, helping to avoid emergency room visits and hospitalizations.
